ABSTRACT

Classical famous prescription Dihuang Yinzi is widely used in modern clinical practice,and can treat many kinds of diseases,especially the diseases of nervous system in internal medicine. Its clinical effect is accurate,but it has not been converted into Chinese patent medicine preparations. Therefore,the authors have collected ancient traditional Chinese medicine(TCM) literatures of Dihuang Yinzi by the methods of bibliometrics,and selected and sorted out 254 pieces of effective data, involving 144 ancient books of TCM,and systematically summarized and analyzed the historical development origin,main treatment syndrome,formula making principle,dosage,preparation method,decoction method and medicine taking method of Dihuang Yinzi,in order to provide the ancient literary evidence support for the development and clinical application of classic famous prescriptions. It is found that Dihuang Yinzi was from Xuanming Lunfang written by LIU He-jian,a doctor of Jin dynasty. It was composed of 12 kinds of herbs,namely Rehmanniae Radix Praeparata,Morindae Officinalis Radix,Corni Fructus,Cistanches Herba,Dendrobii Caulis,Aconm Lateralis Radix Praeparaia,Schisandrae Chinensis Fructus,Cinnamomi Ramulus,Poria,maimendong,Acori Calami Rhizoma and Polygalae Radix, and mainly used for the treatment of Yinfei. The later records of Dihuang Yinzi mostly followed the prescription composition and main treatment set forth in Xuanming Lunfang,and its clinical application was expanded. In the 199 articles with the indications for disease treatment,Yinfei was the most commonest indication, and took up about half of the total,which was followed by stroke,taking up about two fifths of the total. It was also used for the treatment of sudden aphonia,flaccidity syndrome,vertigo,enuresis. Dihuang Yinzi has a wide range of treatment,but the pathogenes is always belongs to "the deficiency of water and fire in the kidney". The recipe of Dihuang Yinzi was unique,and can be used to treat both the upper and lower parts of the body,as well as both the outward symptoms and root causes of an illness at the same time, in particular,it mainly focuses on the treatment of the lower and the root. Among the 56 literatures with drug dosage records,about one third of them inherited the records of Xuanming Lunfang: "Equal division,the top is the end,3 qian for each dose." The dosage was generally light. The preparations are mostly decoction and boiled powder. In the decocting and taking methods,it was suggested that "turbid medicine shall be boiled for a short time,and taken after several boilings,with no limit to time."

ABSTRACT

Objective To understand the epidemic situation and the source of infection of the reemerge human rabies in Qinghai. Methods We collected the data on human rabies and the data on the cases of multi- victims bitten by the identical dog, and also the laboratory data of the nucleoprotein ( N) gene of rabies virus from the samples which were detected by reverse transcription-polymerase chain reaction (RT-PCR) and direct immunofluorescence assay (DFA) from 2012 to 2017, to describe the epidemiological characteristics of human rabies and the prevalence of rabies virus in host animals, and to explore the source of infection of reemerge human rabies. Results A total of 7 human cases were reported in 2012-2017 in Qinghai province, among which 1 was bitted by wolf, 2 were bitted by stray dogs, 3 were bitted by domestic dogs which injured by stray dogs or wolfs. A total of 892 canine brain tissue samples were collected, from which 46 positive samples were detected with the positive rate of 5.16% (95% CI:3.70%-6.61%). The positive samples were collected from the nomadic region, which were consistent had the location of the human rabies. The samples collected from the cases of multi-victims bitten by the identical dog/animal had the positive rate of 73.08%, and 4 out of 7 human rabies were exposed to the cases of multi-victims bitten by the identical dog/animal. Genetic sequencing of the rabies virus detected from canine brain tissue samples were belong to China IV lineage, which was closely related to the Arctic clade. Conclusions The reemerging rabies happened in nomadic region of Qinghai province could be a consequence of spillover from wildlife especially from wolfs. The better surveillance system covering the human, livestock and wildlife should be set up to mitigate the rabies virus spread from the wildlife.

ABSTRACT

·AIM:To investigate and analyse the prevalence and risk factors associated with diabetic retinopathy severity in Qingdao. ·METHODS: This survey consisted of the 2 following parts: 2859 community residents aged >60 years old and 4275 patients with T2DM who were older than 30 years old in Qingdao. Ophthalmic examinations were performed on all patients. A questionnaire was used to obtain the patient's age and gender, the duration of diabetes mellitus(DM), glycaemic control and their knowledge of diabetic retinopathy ( DR ). Blood pressure and haemoglobin levels were recorded. All included patients underwent a comprehensive ophthalmic examination that included a fundus examination and retinal photographs and that assigned a grade for the severity of retinopathy according to the Early Treatment Diabetic Retinopathy Study (ETDRS) severity scale. Patients with severe non-proliferative or proliferative diabetic retinopathy and clinically significant macular edema ( CSME ) required ophthalmic therapy were assigned to the need-treatment group, while the remaining patients with DR were assigned to the need-observation group. Correlation and regression analyses were performed to determine the required-treatment rate and risk factors for DR. Logistic regression models were used to estimate odds ratios (OR) and 95% confidence intervals (CI) after adjustment for age, gender and the duration of diabetes. ·RESULTS: DR was present in 334 (11. 68% ) of the 2859 community residents aged > 60 years old and 1097 (25. 66% ) of the 4275 hospital patients with T2DM, and 48 (14. 81% ) of the residents and 172 ( 15. 68% ) of the hospital patients required ophthalmic therapy. In univariate and multivariate logistic analyses, factors including the age of the patients (51-60 years old: OR, 1. 68; 95% CI, 1. 21-1. 72; 61-70 years old: OR, 1. 55;95% CI, 1. 38-1. 76), the duration of diabetes (11-15 years:OR, 2. 61; 95% CI, 1. 51-4. 72; >15 years: OR, 4. 15; 95% CI, 2. 32-5. 77), glycaemic control (medium: OR, 2. 51;95%CI,1.98-3.92;poor:OR,4.69;95%CI,3.39-6.95), and knowledge of DR ( did not understand: OR, 1. 45;95%CI, 1. 21-1. 95) were significantly associated with the required-treatment rate in DR, while gender, low and advanced age ( 31-50 years old and >70 years old ), duration of disease (<10y), hypertension, and insulin treatment did not. ·CONCLUSION: The prevalence rate and the required-treatment rate in DR in Qingdao are relatively high. Being aged 51-70 years old and having a duration of diabetes>10y, poor glycaemic control and a lack of knowledge of DR were found to be potential risk factors that increased the rate of required ophthalmic therapy in patients with DR. In patients with T2DM who were aged 51-70 years old, we found that focusing on using science and education to strengthen the patients' knowledge of DR, establishing specifications for a community DR screening system, and effectively implementing early intervention in the community of DR - affected individuals were particularly important for preventing and controlling the high DR prevalence and the high rate of DR-associated blindness

ABSTRACT

AlM: To compare the results of 4 methods for measuring angle of exodeviation in the three types of intermittent exotropia, including when looking at indoor distance target of 6m, looking at indoor distance target of 30m, looking at outdoor far distance target, after 1h diagnostic occlusion test. METHODS: Prospective case series study. Sixty-five patients with intermittent exotropia between June 2013 and June 2014 were enrolled in the Department of Ophthalmology, Affiliated Hospital to Qingdao University, included 37 males and 28 females with average age ( 12. 5 ± 6. 2 ) years. All the patients were measured when looking at indoor distance target of 6m, looking at indoor distance target of 30m, looking at outdoor far distance target, after 1h diagnostic occlusion test. lntermittent exotropia was divided into basic type, convergence insufficiency type and divergence excess type, which was based on the different result of between the distance and near measurements. The One-way test was applied to analyze the four methods of measuring angle of exodeviation in the three types of intermittent exotropia. LSD - t test was applied to compare the differences between each two methods in each type. RESULTS: The distance exodeviations tested with looking at indoor distance target of 6m, looking at indoor distance target of 30m , looking at outdoor far distance target, after 1h diagnostic occlusion test were basic type (45. 4 ± 21. 0, 55. 0 ± 15. 0, 64. 68 ± 17. 7, 68. 75 ± 16. 6PD), convergence insufficiency type (33. 3 ± 14. 0, 44. 9 ± 12. 9, 43. 6±11. 8, 54. 6±11. 2PD), divergence excess type (55. 6± 17.4, 66.3±18.8, 76.9±16.4, 78.1±15.6PD). There were obviously differences between each two methods in each type ( basic type F = 9. 649, P = 0. 00; convergence insufficiency type F=6. 886, P=0. 001; divergence excess type F = 7. 989, P = 0. 00 ). Compared with looking at indoor distance target of 30m, looking at outdoor far distance target ( basic type P=0. 044, divergence excess type P = 0. 048 ) and after 1h diagnostic occlusion test (basic type P=0. 04, divergence excess type P=0. 027) had the statistical difference in the basic type and divergence excess type, and there was no obviously difference between looking at outdoor far distance target and after 1h diagnostic occlusion test ( basic type P=0. 353, divergence excess type P=0. 815). Compared with the other three measurements, 1h diagnostic occlusion test can elicit larger angle of deviation in the convergence insufficiency type. CONCLUSlON: Both measurement with looking at outdoor far distance target and after 1h diagnostic occlusion test can elicit the larger angle of deviation in the basic type and divergence excess type; The measurement with after 1 hour diagnostic occlusion test can elicit the larger angle of deviation in the convergence insufficiency type.

ABSTRACT

Background The occurrence of experimental myopia may be related to glucagon,and within the range of certain concentration,glucagon may inhibit the development of myopia,but its exact action mechanism is not completely clear.Objective Purpose of this study was to evaluate the effects of glucagon on the proliferation of guinea pig scleral fibroblast cells(GSFCs) and the possible role of glucagon in myopic scleral remodeling.Methods The scleral tissue was obtaincd from the clean blooded guinea pig aged 15 days.GSFCs were cultured and identified with vimentin antibody,cytokeratin antibody and S-100 antibody.0,5,10,50,100,200 μg/L glucagon was added into the different cultured hole for 24 hours respectively,and the growth and proliferation (A490 value) of GSFCs was detected by MTT colorimetric assay.Then the A490 value of GSFCs was assayed in 1 day,2,3,5,7 days under the 50 μg/L glucagon action.Matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-2 (TIMP-2)levels(A450 value)in GSFCs were detected by enzyme-linked immuno sorbent assay (ELISA)72 hours after the cells cultured.Results Passaged GSFCs showed the dendric array at lower density and gyrate array at the higher density with the positive response for vimentin.A490 values of GSFCs were gradually increased with the rise of glucagon concentration(F=32.340,P=0.013).When the glucagon concentration was 10-200 μg/L,the A490Value of GSFCs was higher than that without glucagon group,showing signifitant differences between them(t =5.575,6.627,16.074,12.003,P<0.05).Under the 50 μg/L glucagon action,A490 values were significantly accented with the time lapse (Ftime =10.610,P =0.024),and the A490 values also were significantly higher than the parallel control groups without glucagon(Fgroup =9.068,P=0.039).MMP-2 level was gradually declined with the enhance of glucagon within range of 5-200 μg/L(F=153.639,P=0.036),but no significant difference was found in TIMP-2 expression(F=24.770,P=3.250).Conclusions Glucagon can promote the proliferation of GSFCs in vitro,and the synthesis of MMP-2shows a concentration-and time-dependent manner.

ABSTRACT

AIM: To evaluate the effect of binocular visual function training in pediatric ocular trauma. METHODS: There were 76 patients (76 eyes) that were hospitalized with a primary diagnosis of ocular injury at the Affiliated Hospital of Medical College, Qingdao University between January 2006 and December 2009. Binocular visual function training was given after primary wound repair. Far stereopsis function were checked using AIT-1000 synoptophore fusion, and near stereopsis function was checked using Titmus stereogram.Binocular visual function was compared before and after training. RESULTS: Before binocular visual function training,26 eyes(34%)had no binocular vision, after training there were only 16 eyes(21%) without binocular vision. Before undertaking binocular visual function training with fusion, only 27 eyes (36%) had binocular vision, after the training there were 48 eyes (63%) with binocular vision. Before undertaking binocular visual function training with far stereopsis ,there were 23 eyes (30%) with binocular vision, after the training there were 29 eyes (38%). Before binocular visual function training with near stereopsis, there were 14 eyes (18%) with binocular vision, after the training there were 33 eyes (43%) with binocular vision. There was a significant difference in the number of patients with binocular vision before and after binocular visual function training. CONCLUSION:The training is useful for the reconstruction of binocular visual function in pediatric ocular trauma.

ABSTRACT

Background The domestic and international researches discovered that many proteins and enzymes of the extracellular matrix (ECM) participate in the sclera remodeling by affecting the collagen typeⅠand fibronectin.Objective This study was to investigate the effect of matrixmetalloproteinase-2 (TIMP-2) on expression of collagen typeⅠand fibronectin of ECM in the posterior sclera by injecting liposomes containing tissue inhibitor of TIMP-2 gene into suprachoroidal space of the form-deprivation myopia in guinea pig.Methods Form-deprivation myopia was induced by translucent goggles in 36 clean guinea pig for 2 weeks.Then the animals were randomly assigned to TIMP-2 group,empty plasmid group,saline group and 12 for each group.Liposomes of 5μl containing TIMP-2 gene,empty plasmid and saline were suprachoroidally injected in the right eye respectively,and the left eyes without any treatment were used as self-control group.Other 12 matched guinea pigs only covered the right eyes through out the experimental duration as model control group.The guinea pigs were sacrificed and the posterior sclera tissue of the eyeballs were collected at 2,7 and 14 days after injection of drug.The expressions of collagen typeⅠmRNA and fibronectin mRNA were detected by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR).This study followed the Regulation for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission.Results The expression level of collagen type Ⅰ mRNA in the posterior sclera of guinea pig was lower but that of fibronectin mRNA was higher in TIMP-2 group than self-control group,showing significant differences between them (P<0.05).The expression level of collagen type Ⅰ mRNA in the posterior scleral tissue began to increase from the 2nd day after drug injection and was obviously elevated at the 7th day and then gradually decreased at the 14th day.However,the expression level of fibronectin mRNA in the posterior scleral tissue showed the opposite pattern.The expression levels of collagen typeⅠmRNA and fibronectin mRNA at the 7th after drug injection were significantly lower than that at the 2nd day or 14th day (P<0.01).Conclusion Suprachoroidal injection of TIMP-2 in form-deprivation myopia could up-regulate the expression of collagen typeⅠmRNA and down-regulate the expression of fibronectin mRNA in the posterior scleral tissue.It may slow down the sclera remodeling of form-deprivation myopia in guinea pig in the early stage.

ABSTRACT

AIM: To investigate the postoperative changes of binocular visual function in patients with congenital superior oblique palsy.METHODS: Eye position and binocular visual function were examined in 28 patients with congenital superior oblique palsy pre- and post-operatively. The results were statistically analyzed.RESULTS: The normal eye position rate after operation was 89.3%. There were 9 patients who gained simultaneous perception after operation, while 13 patients gained fusion as many as that gained stereoacuity. There was statistically significant difference between postoperative and preoperative eyes (P<0.05). Near stereoacuity was improved after operation. Patients who had better visual acuity and low strabismus degrees as well as fusion had a better recovery.CONCLUSION: The operation could improve the eye position and rebuild the binocular visual function of the congenital superior oblique palsy. The recovery of binocular visual function was related to visual acuity and strabismus degrees of the patients and whether they had fusion function.

ABSTRACT

<p><b>OBJECTIVE</b>To study the genetic characterizations of VP1 gene of human enterovirus 71 (HEV71) isolated from clinical specimens of Hand, Foot and Mouth Disease (HFMD) patients in Qinghai Province in 2008.</p><p><b>METHODS</b>335 clinical samples including stools, throat swabs and vesicle fluids were collected from HFMD patients in Qinghai Province. Viral isolation was performed, and molecular typing was performed with the positive isolates. Then 30 identified HEV71 isolates were performed for entire VP1 coding region amplification and sequencing.</p><p><b>RESULTS</b>Among the 355 clinical samples, 45 human enteroviruses were isolated, and among them, 30 were identified as HEV71. Then 30 HEV71 positive isolates were performed by nucleotide sequencing. It showed that there was some difference in the nucleotide and the amino acid among the 30 HEV71 strains, the homology were 95.2%-100% and 96.6%-100%, respectively. But they all closed to HEV71 strains isolated in China after 1998, and from the phylogenetic tree constructed with 30 Qinghai HEV71 strains and other 35 HEV71 strains represented all known genotype and subgenotype HEV71 strains available from GenBank, it revealed that the 30 Qinghai HEV71 strains clustered within the C4a evolution branch of C4 subgenotype.</p><p><b>CONCLUSION</b>HEV71 was isolated in HFMD patients in Qinghai province, and the HEV71 strains causing HFMD outbreaks in Qinghai province in 2008 were all belong to C4a evolution branch of C4 subgenotype with several transmission chains.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of ultrasonic irrigation in deactivating and eliminating the endotoxin in the root canals of chronic periapical periodontitis.</p><p><b>METHODS</b>10 maxillary premolars with 2 root canals extracted because of serious chronic periapical periodontitis were opened conventionally and prepared with modified double-flared technique with apical foramen sealed. 30 microL 3% H2O2 solution was introduced into each root canal and 2 minutes ultrasonic vibration was given. Samples were taken before and after ultrasonic vibration. 10 maxillary premolars with 2 root canals of chronic periapical periodontitis of 10 patients in clinic were selected. The root canals of them were instrumented as above, followed by 2 minutes ultrasonic irrigation with asepsis distilled water. Samples were taken before, after root canal preparation and after ultrasonic irrigation. The endotoxin activity of them was tested by kinetic turbidimetric limulus test.</p><p><b>RESULTS</b>The mean endotoxin activity of extracted premolars before ultrasonic vibration was 4.069 EU x mL(-1) while after ultrasonic vibration it was 16.410 EU x mL(-1). There was great statistical difference between them (P < 0.01). The endotoxin activity after ultrasonic vibration was significantly higher than that of before vibration. The clinically mean endotoxin activity before, after root canal preparation and after ultrasonic irrigation were 44.860, 4.099, and 0.116 EU mL1 respectively. There was great statistical difference between the endotoxin activity before and after root canal preparation (P < 0.01). After the preparation, the endotoxin activity droped obviously. There was great statistical difference between the endotoxin activity before and after ultrasonic irrigation (P < 0.01). After ultrasonic irrigation, the endotoxin activity droped obviously.</p><p><b>CONCLUSION</b>Under the condition of simulating clinical work presence, the ultrasonic irrigation can not intensify the effect of the root canal rinse solution in deactivating the endotoxin of infected root canals, but it may intensify the effect of the root canal rinse solution in eliminating the endotoxin of infected root canals.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the deactivation of the endotoxin in artificial glass root canals with ultrasonic vibration.</p><p><b>METHODS</b>80 artificial glass root canals were randomly divided into 8 groups: Ultrasonic vibration of 5 minutes, 7 minutes, 10 minutes, ultrasonic vibration of 5 minutes together with 3% H2O2 solution, only 3% H2O2 solution, ultrasonic vibration of 5 minutes together with 5.25% NaClO solution, only 5.25% NaClO solution and the control. Standard endotoxin solution was introduced into each root canal. Different time's ultrasonic vibration was applied to different groups. After ultrasonic vibration, the endotoxin activity of each group was tested by kinetic turbidimetric limulus test.</p><p><b>RESULT</b>There were no significant differences among the groups of different time ultrasonic vibration and the control (P>0.05). There was great statistical difference between each group with root canal rinse solution and the control (P<0.001). The endotoxin activity of the test was significantly lower than the control. There was no significant difference between the groups of only rinse solution and rinse solution together with ultrasonic vibration.</p><p><b>CONCLUSION</b>Under the condition of this study, the only ultrasonic vibration can not deactivate the endotoxin of infected root canals and can not intensify the effect of root canal rinse solution.</p>

ABSTRACT

NAD +-dependent cytosolic glycerol-3-phosphate dehydrogenase in Sacch aromyces cerevisiae is one of the key enzymes in metabolic pathway of glycerol . catalysing the reduction of dihydroxyacetone phosphate to glycerol-3-phosph ate.It has two isoenzymes.To study the differences between their structures, their expression of encoding genes and their functions may help increase the understan ding of the cell response mechanism to the hyperosmotic and anoxic conditions. In this paper the research on the two isoenzymes was reviewed.
